After a referral is received from your doctor or hospital case manager, our compassionate and experienced therapist will go to your home, evaluate your needs and tailor a comprehensive rehabilitative care plan to help manage your pain, and maintain safety and independence at home.
Our team of therapists can help you regain functional independence after surgery, stroke, injury, or illness, by helping you develop the physical skills for daily living such as bathing, brushing teeth, cutting meat and dressing.
Our speech therapists help you make improvements in speech and language, swallowing, and oral motor voice.
NEW: PRE SURGERY PHYSICAL THERAPY PROGRAM
Prior to your surgery, our physical therapist visits your home to conduct a Home Safety Evaluation. At that visit, she will develop a plan that includes safe furniture placement, as well as educating you on what to expect when you return home from the hospital. This program improves your level of post-operative healing.
