New Milford Visiting Nurse & Hospice is dedicated to serving seniors who want to continue to live happily and independently, even as health challenges arise and their care needs change.

After a referral is received from your doctor or hospital case manager, our compassionate and experienced nurse will go to your home, evaluate your needs and tailor a care plan that may include oncology care, wound care, medication teaching, diabetes education, disease and pain management, and preventative care instruction.

We take care of you in your home and if you are admitted into the hospital or rehabilitation center, we help transition you back home with our nurses, therapists and caregivers from our private duty home care provider VNA HOME.

Adult Health Promotion Program

PROGRAM GOALS:
To provide health promotion and health education to individuals with the following outcomes: Adequate nutrition and fluids to promote health, an environment that promotes client and home safety, medical intervention at the first signs of acute illness, physician follow-through and health promotion to prevent hospitalization.

SERVICE PLAN:
Nursing visits approximately every 1 to 3 months to evaluate patient status, assess vital signs and nutritional status, observe and assess patient/home safety, assess patient support systems, provide names of community resources and instruct the importance of follow up with the physician.

Remote Patient Monitoring (Telehealth)

New Milford VNA and Hospice offers daily remote patient monitoring. Remote patient monitoring is a way for the agency to monitor your blood pressure, heart rate, oxygen level, and weight on a daily basis. A simple device will be set up in your home for you to check your vital signs every day and the results will be sent to New Milford VNA and Hospice electronically. Our compassionate nurses will check your results every day and be in contact with you, as well as your doctor, if we have any concerns. This is an excellent way to assist in keeping you home and safe, while having daily monitoring of your health status.
