Our commitment to our patients is guided by a team of community nurses, therapists and social workers. We take care of you at your home, and if you are admitted into the hospital or rehabilitation center, we help transition you back home with our nurses, therapists, social workers and aides.

After a referral is received from your doctor or hospital case manager, our compassionate and experienced nurse will go to your home, evaluate your needs and tailor a care plan that may include oncology care, wound care, medication teaching, diabetes education, disease and pain management, and preventative care instruction.

Working under the direction of our nurses and therapists, our Home Health Aides (HHAs) help those in our care with personal care activities as dressing, eating, bathing, shopping, and light housekeeping. These services are offered through both New Milford VNA & Hospice and our sister company, VNA Home,that offers private pay services. In addition to medical care, counseling and coordination of community services, we are provided to families and individuals as needed to help them cope with problems associated with illness or the care of a loved one, we manage ongoing communication with all the members of the caregiving team, including physicians, financial and legal advisors and family member.
