Qualifications:

A licensed registered nurse with BSN or MSN with three to five years health-related experience and one-year of progressively responsible management experience in Home Care required.

Strong oral, written, analytical and organizational skills required.  Excellent communication skills required.

Through knowledge of current federal and Connecticut state laws, regulations, certification requirements pertaining to Home Health required.

Basic Function of Job:

Responsible for clinical management of the Home Health Agency, including assessing, planning, implementing and evaluating delivery of patient care in the home setting. May assume primary supervisory responsibility for one or more clinical teams.  Also provides administrative direction to development and evaluation of clinical and clerical personnel.  Performs duties and responsibilities in a manner consistent with corporate values. Works closely with Home Health Manager in assuring excellent in service to patients while maintaining regulatory compliance.

Major Duties and Responsibilities:

Manages home care clinical services.  Duties include:

Developing, planning and coordinating patient services, delegating responsibilities, as appropriate.

Establishing clinical goals and objectives, ensuring understanding of and commitment to goals among staff.

Ensuring that Agency clinical care and documentation complies with all federal and Connecticut state laws, regulations, certification requirements and standards pertaining to home care.

Monitoring and assuring compliance with all clinical policies and procedures, providing interpretation as needed.  Assists with development and revision as indicated.

Management of Personnel.  Duties include:

Ensuring adequate hiring, orientation, evaluation, guidance and counseling of clinical staff and schedulers if appropriate, the schedulers.

Maintaining staffing requirements appropriate to case load and budget.

Participating in the hiring process for new staff, and assuring compliance with appropriate regulations and policies.

Providing for professional growth and development of the home care staff and establishes climate conducive to meeting educational needs and learning objectives.

Supervising all assigned field staff of designated territory assuring compliance with physician orders and patient care practices; accurate and complete documentation.  Assesses field staff care through visits to patient’s home, and coordinates and assists with visit schedules.

Recommending an acceptable plan for patient care when field staff requests assistance or when the documentation of a problem requires the addition of a care plan.

Insuring that Agency policies and procedures are followed.  Coordinates and assures compliance with documentation of case conferences.

Assuring that services provided through contracts with agencies are properly executed, negotiating new contracts as appropriate.

Fiscal Responsibilities. Duties Include:

Serving as a resource and interpreting Medicare/third party reimbursement guidelines, ensuring proper documentation.

Monitoring and assuring productivity and labor at standard levels

Communication/Customer Satisfaction.  Duties include:

Fostering and mentoring a positive attitude and loyalty towards the organization, home care and job responsibilities.  Demonstrating positive interpersonal relations in dealing with patients, families, volunteers, physicians, fellow employees and other customers so as to promote efficiency and customer satisfaction.

Maintaining proper relationships with doctors, Hospitals, Home Medical Equipment, Private Duty, other health care providers, etc., delegating responsibility for contacts as appropriate.

Managing patient complaints resolution in a positive and timely manner.  Assures follow-up until resolution is determined.

Actively participates in quality management activities to improve patient and customer service.

Performs other management functions as needed.  Duties may include:

Working closely with Director of Clinical Services and Executive Director/CEO to assure all branch functions are coordinated and in compliance with regulations.

Performs other duties as assigned.
